The Children’s Wraparound Community Based Family Specialist provides community-based counseling services to children and their families. This position works collaboratively with the clinical team and state providers to support the child and family in working towards and completing their recovery goals outlined in the treatment plan. This is a full-time, 35 hour a week position.
This position works with children, adolescents and families working in partnership in the System of Care Fast Forward program (SOC-FF).The NH System of Care is a value-based system designed to serve children, youth and families experiencing difficulties due to the child or youth's Severe Emotional Disturbance. The child or youth served through the system of care are at risk for acute psychiatric hospitalization or out of home placement in a residential treatment facility.
The Community Support Family Specialist will assist struggling families become successful by supporting and teaching values and skills necessary to function and achieve their personal goals. Counseling interventions will be based in the community setting such as home, school and other social environments.
Major Functional Areas and Description of Duties:
Working with children and their families to provide direct support within the community;
Document and articulate treatment interventions effectively;
Communicate symptoms, behaviors, strategies and interventions to the clinical team;
Maintain professional codes of conduct and ethics;
Perform any additional duties assigned by supervisor.
Physical Demands and Working Conditions:
The majority of the work takes place in the community. The ability to actively engage with children and conduct interactive play within a therapeutic setting is required. Manual dexterity is needed to use computers and standard office equipment. The ability to sit for extended periods of time is required. Reliable transportation is required, along with a valid driver’s license and a clean driving record, as home visits to clients are expected.
