The Clinical Case Manager provides outpatient clinical/case management services to CLM – Community Support Program (CSP) clients who are in need of community supports. CSP Clinical Case Manager services include the quarterly assessments and reviews, eligibility determination, and treatment plans of a CSP client and along with limited case management. This is a full time, 35 hour per week position.
Major Functional Areas and Description of Duties:
Provide assessments on a quarterly basis to CSP adults;
Develop eligibilities and treatment plans with each client; specify presenting problem, objectives, interventions, and desired outcomes;
Assess for/and make referrals to CSP such as Supported Employment, Illness Management and Recovery evidence based practices;
Provide timely documentation on each service delivered;
Collaborate and coordinate efforts with CSP treatment team and other involved providers;
Work with clients in their homes and in the community to provide functional support services and thorough assessments.
Physical Demands and Working Conditions :
Must have ability to sit at computer and use keyboard for data entry;
Must be able to flex schedule to accommodate needs of clients and be responsive to office space issues of agency;
Must have reliable transportation and the ability to travel within the community on a daily basis;
Must have ability to sit for long intervals while working with clients;
Must be willing to enter the community to provide in-home sessions.
